TAXI DRIVER HISTORY:
Taxi drivers are under ethical obligations. In assessing whether it is appropriate to allow you to driver for STS, it is important to consider your suitability by reference to those ethical obligations including by having regard to your character and prior conduct.
The following questions are important in assisting to assess your suitability to driver for STS.
2.1 Have you ever been found guilty or received a fine by Accreditation and Licensing Centre (ALC) or been banned from driver for a transport booking service (e.g 13Cabs, Uber etc)? *

I am accredited by the Department of Planning Transport & Infrastructure (DPTI), & hold a current SA Driver's Licence and conform in all aspects with the Passenger Transport Act 1994 and the Road Traffic Act 1959.
I am in receipt of the STS Handbook for the Accredited Operator and Accredited Driver. I agree to be bound by the policies and rules as set out from time to time by STS as outlined.
I am aware of this handbook's reference to the Disability Discrimination Act (DDA) 1992 and Equal Opportunity Act 1984. I also acknowledge reference is made to the Taxi Licence Conditions and Schedule 5 - Code of practice of the Passenger Transport Regulations (PTR) 2009 to 1.07.2015.
I acknowledge that Section 68 (30) (f)& 68 (5) of the Passenger Transport Regulations 2009 makes it compulsory to transport guide dogs, hearing dogs or other approved assistance animal with their owner. I understand that neither the DDA 1992 nor the PTR 2009 allow for any reason to refuse carrying a guide dog, hearing dog or any other assistance animal.
I agree that I must always be logged into the Mobile Data System and I must log on using my allocated Personal Identification Number (PIN) at the commencement of the shift and I must log off at the end of the shift. I must not give my PIN to any other driver or Taxi Operator.
I will always respect the confidentiality of all information offered to me for the provision of services and will not use any information in any, way other than for the sole purpose of carrying out the service.
I understand that my PIN will expire when my current Taxi Driver's Licence expires and I must re-apply for my PIN with my new Licence.
I cannot and will not give a Job that has been dispatched to me by the STS's Taxi Network to another driver. If I cannot do the job, then I can only resubmit the job to the STS's Taxi Network.
In the event that there is a dispute between myself and the STS, I acknowledge that the first point of contact Is between myself and the Supervisor. If the dispute is not resolved to the satisfaction of either party then the Manager can be involved to try and resolve the dispute.
I accept that the STS and/or its officers can suspend my PIN and hence myself from doing any work within the STS's Taxi Network.
